Enrollment Process

To enroll your child, please follow these steps:

1. Pre-registration by the child's guardians.
2. Submit the following documents:
   • Completed enrollment form
   • Child's individual health booklet
   • Identification of the attending physician
   • Identification of authorized persons to pick up the child and their relationship to the child
   • Service provision contract
